Position Overview

We are seeking a dedicated Clinical Pharmacist to join a collaborative hospital pharmacy team focused on delivering safe, effective, and patient-centered medication therapy. This role partners closely with physicians, nurses, and other healthcare professionals to optimize medication use, improve patient outcomes, and support high-quality pharmaceutical care across a variety of inpatient clinical settings.

The ideal candidate has recent hospital clinical pharmacy experience, strong critical thinking skills, and a passion for improving medication safety and patient care through evidence-based practice.

Key Responsibilities

• Provide comprehensive clinical pharmacy services and direct patient care across assigned service lines.
• Review medication orders to ensure safe, effective, and evidence-based therapy.
• Monitor patient medication regimens and identify opportunities to optimize treatment outcomes.
• Collaborate with physicians, nurses, and interdisciplinary care teams to develop individualized medication plans.
• Perform medication reconciliation, discharge medication review, and patient counseling as appropriate.
• Identify, prevent, and resolve medication-related problems, adverse drug events, drug interactions, and dosing concerns.
• Participate in antimicrobial stewardship, medication utilization initiatives, and other clinical pharmacy programs.
• Support pharmacy operations, medication dispensing, and decentralized pharmacy services as needed.
• Contribute to quality improvement initiatives, regulatory compliance activities, and medication safety programs.
• Monitor pharmacy programs and recommend operational or clinical improvements based on best practices and performance data.
• Assist with education and training of pharmacy students, residents, and clinical staff.
• Maintain accurate clinical documentation and utilize hospital pharmacy systems effectively.
• Support multidisciplinary teams in delivering high-quality, patient-centered care.

Qualifications

• Graduate of an accredited College of Pharmacy required; PharmD preferred.
• Active Iowa Registered Pharmacist license required.
• 2–4 years of clinical pharmacy experience in a hospital setting required.
• Strong knowledge of hospital pharmacy practice, medication management, and clinical pharmacotherapy.
• Excellent written and verbal communication skills.
• Strong critical thinking, clinical judgment, and problem-solving abilities.
• Ability to prioritize multiple responsibilities while working independently and collaboratively.

Preferred Qualifications

• Completion of an ASHP-accredited PGY-1 Pharmacy Residency.
• Board Certified Pharmacotherapy Specialist (BCPS) certification.
• Experience participating in quality improvement, medication safety, or clinical practice initiatives.
• Experience serving as a preceptor or mentoring pharmacy students and residents.
