Clinical Pharmacist – Managed Care

Location: Portland, OR 97213 (Remote)
Duration: 26 Weeks
Shift: Day Shift
Hours: 8:00 AM – 5:00 PM | Monday–Friday | 40 Hours Per Week

We are seeking a highly skilled Clinical Pharmacist to support managed care pharmacy services in a remote setting. The ideal candidate will have strong experience in medication therapy management, prior authorization review, utilization management, formulary support, and evidence-based clinical decision-making.

Requirements:

  • Doctor of Pharmacy (PharmD) required.

  • Active Pharmacist License in Oregon, Washington, or California required.

  • Completion of PGY-1 Residency required.

  • Minimum 3 years of managed care pharmacy experience required.

  • Experience with prior authorization reviews, appeals, and utilization management.

  • Strong knowledge of clinical pharmacology, therapeutics, and evidence-based medicine.

  • Experience with pharmacy claims systems and data analysis tools.

  • Strong communication, analytical, and problem-solving skills.

  • Ability to work independently in a fast-paced remote environment.

Responsibilities:

  • Provide Medication Therapy Management (MTM) services and identify medication-related issues.

  • Review pharmacy and medical claims for clinical appropriateness and safety.

  • Conduct prior authorization reviews, drug utilization reviews (DUR), and appeals.

  • Develop patient-specific medication action plans to improve outcomes.

  • Collaborate with physicians, nurses, case managers, and healthcare teams.

  • Participate in Pharmacy & Therapeutics (P&T) Committee activities and policy development.

  • Support population health and disease management initiatives.

  • Monitor and evaluate clinical outcomes of pharmacy interventions.

  • Provide evidence-based drug information and recommendations.

  • Assist with formulary management, medication policies, and utilization programs.

  • Participate in quality improvement and regulatory compliance initiatives.

  • Educate providers, staff, and members on medication-related topics.

  • Support managed care operations and pharmacy program initiatives.
