POSITION SUMMARY: Identifies, resolves, and prevents drug-related problems. Recommends and monitors patient drug therapy and provides drug information. Interacts with patients, family, and other health care providers as appropriate to assure continuity of services and to achieve desired patient outcomes through safe, timely, cost-effective drug therapy. Participates in formal and informal activities that enhance professional development, training of students and residents, and clinical research.
JOB SPECIFIC DUTIES:
A. Prepares and dispenses medication orders per physician request according to established policies,
procedures, and protocols.
1. Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles
accurately.
a. Maintains accurate, complete patient medication profiles.
b. Calculates chemotherapy, parenteral nutrition, pediatric doses and continuous infusions accurately. Performs
required double checks on chemotherapy and pediatric dosing.
2. Accurately compounds and dispenses medications.
a. Labeling follows departmental procedures
b. Products are bar-coded according to departmental procedures
3. Identifies and selects specific dosage forms as appropriate for all ages of patients served.
4. Assures compliance with organizational policy regarding home medication, self-administration and bedside storage
of medications
5. Controlled substances:
a. Resolves discrepancies and appropriately notifies pharmacy and hospital administration for unresolved
discrepancies or suspected diversion.
b. Issue controlled substance to patient care areas and maintains records as required by law.
c. Knowledgeable of the process and/or correctly and accurately receives schedule II controlled drugs and
oversees the pharmacy technician receiving all controlled substances.
6. Sterile products:
Chemotherapy/hazardous drugs
Parenteral nutrition (TPN)
Small and large volume parenterals
Other sterile products such as eye drops and irrigation solutions
a. Cleans, operates and maintains the laminar air flow and hoods and biological safety cabinet according to
departmental procedures
b. Assures routine cleaning of room and surfaces following departmental procedures
c. Wears appropriate garb and washes hands according to departmental procedures
d. Demonstrates aseptic technique and successfully passes testing for process validation (media fill test)
e. Disposes of sharps and hazardous waste according to organizational procedures
f. Prepares sterile products according to departmental procedures
g. Accurately dispenses and labels sterile products
7. Crash carts and emergency medications:
a. Assures refill procedures are accurate and completed prior to end of shift and as needed on evening and night
shifts. Required documentation is accurately completed.
b. Assures that new crash carts are delivered within 15 minutes of code notification.
8. Investigational medication record keeping and dispensing follows the protocol and departmental procedures
B. Ensures safe, appropriate, cost effective medication therapies for patients according to established
policies, procedures and protocols.
1. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-
disease state interactions, allergies, and appropriateness of drug and dose.
2. Assists with pharmacokinetic consult service and drug dosing per organizational protocol
3. Assists with anticoagulant consult services and drug dosing per organizational protocols
