    Position Summary

    LifeBridge Health is seeking a Clinical Performance Coordinator to support our primary care practices in advancing high-quality, value-based care delivery. This role is instrumental in improving patient outcomes through proactive patient engagement, care coordination, quality improvement initiatives, and clinical performance optimization.

    As a member of the Population Health team, the Clinical Performance Coordinator partners with providers, care managers, practice leaders, and administrative teams to enhance patient engagement, optimize population health strategies, close care gaps, improve transitions of care, and increase completion of Annual Wellness Visits (AWVs).

    This position combines healthcare operations, clinical coordination, and data-driven improvement strategies to support organizational quality goals while enhancing the overall patient experience.

    Key Responsibilities

    Population Health & Patient Engagement

    • Identify and conduct outreach to attributed and unaligned patients to establish and strengthen primary care relationships.

    Transitions of Care

    • Review daily Admission, Discharge, and Transfer (ADT) reports and coordinate timely follow-up appointments following hospitalizations and emergency department visits.

    Quality Improvement Initiatives

    • Utilize payer reports, registries, and electronic health record (EHR) tools to identify gaps in care. Collaborate with clinical teams to improve completion of preventive screenings, immunizations, laboratory testing, chronic disease management, and recommended follow-up care.

    Annual Wellness Visits (AWVs)

    • Identify and Coordinate Medicare patients eligible for Annual Wellness Visits.

    Documentation, Data Tracking & Performance Reporting

    • Assist providers with documentation practices that support quality measures and value-based reimbursement. Monitor key performance indicator and dashboards s related to population health initiatives

    Schedule & Work Location

    Schedule: Full-Time | Monday-Friday, 8:00 AM-4:30 PM

    Work Location: On-site at multiple LifeBridge Health primary care practice locations throughout the Baltimore metropolitan area

    Qualifications

    Education & Experience

    • Associate degree preferred
    • Equivalent combination of education and relevant healthcare experience may be considered
    • Two (2) to four (4) years of experience in healthcare operations, population health, care coordination, medical office workflows, or quality improvement
