Job Summary

The Clinical Manager is responsible for overseeing patient care coordination, clinical operations, and staff supervision in a home health setting. This role ensures high-quality, compliant care delivery, supports clinicians, and drives positive patient outcomes.

Key Responsibilities
  • Oversee patient care coordination and ensure care is delivered per physician orders and plan of care
  • Develop, implement, and update individualized care plans in collaboration with physicians and interdisciplinary teams
  • Review referrals, assess patient needs, and assign appropriate clinical staff
  • Supervise and mentor clinicians to ensure quality patient care and regulatory compliance
  • Conduct case conferences, chart reviews, and performance evaluations
  • Guide clinicians on clinical decision-making, goals, and treatment plans
  • Ensure coverage and clinical support during all operating hours
  • Participate in quality improvement (QAPI) and performance programs
  • Assist with hiring, onboarding, and staff development/training
  • Maintain compliance with agency policies, state and federal regulations, and professional standards

Qualifications
  • Licensed Registered Nurse (RN) in California
  • Home health experience required; leadership or supervisory experience preferred
  • Strong knowledge of CMS regulations, care coordination, and clinical documentation
  • Excellent communication, leadership, and organizational skills
