Job Summary
The Clinical Manager is responsible for overseeing patient care coordination, clinical operations, and staff supervision in a home health setting. This role ensures high-quality, compliant care delivery, supports clinicians, and drives positive patient outcomes.
Key Responsibilities
Oversee patient care coordination and ensure care is delivered per physician orders and plan of care
Develop, implement, and update individualized care plans in collaboration with physicians and interdisciplinary teams
Review referrals, assess patient needs, and assign appropriate clinical staff
Supervise and mentor clinicians to ensure quality patient care and regulatory compliance
Conduct case conferences, chart reviews, and performance evaluations
Guide clinicians on clinical decision-making, goals, and treatment plans
Ensure coverage and clinical support during all operating hours
Participate in quality improvement (QAPI) and performance programs
Assist with hiring, onboarding, and staff development/training
Maintain compliance with agency policies, state and federal regulations, and professional standards
Qualifications
Licensed Registered Nurse (RN) in California
Home health experience required; leadership or supervisory experience preferred
Strong knowledge of CMS regulations, care coordination, and clinical documentation
Excellent communication, leadership, and organizational skills
