Location: Remote/Field-Based Texas (Dallas, Nueces, and Central Service Delivery Areas)
Duration: 12 Months
Schedule: Monday Friday | 8:00 AM 5:00 PM

Pay Range: $30.00 - $33.00 hourly on W2

Job Summary

We are seeking an experienced LTSS Service Care Manager to coordinate and manage care for members enrolled in Long-Term Services and Supports (LTSS). This role is responsible for conducting comprehensive member assessments, developing individualized service plans, coordinating healthcare and community resources, and ensuring members receive quality, cost-effective care.

This is a hybrid field-based position requiring approximately 75% travel to conduct in-person member visits throughout assigned Texas service areas.

Key Responsibilities

  • Conduct comprehensive member assessments through telephonic outreach and in-person visits.
  • Develop, implement, and monitor individualized care/service plans based on member needs.
  • Coordinate healthcare services, community resources, and long-term support programs.
  • Educate members and caregivers on available benefits, services, and healthcare options.
  • Collaborate with providers, interdisciplinary teams, and community partners to ensure continuity of care.
  • Monitor member progress and update care plans as appropriate.
  • Complete all required documentation accurately within established turnaround times.
  • Maintain compliance with regulatory, quality, and organizational standards.
  • Identify and address barriers to care while promoting positive health outcomes.

Daily Responsibilities

  • Travel approximately 75% of the time to complete 2 3 face-to-face member assessments per day.
  • Follow a hybrid schedule consisting of:
  • 3 field days focused on member visits and assessments.
  • 2 documentation days dedicated to care planning, follow-ups, and completing documentation within the required 3-day turnaround time (TAT).
  • Coordinate services, communicate with providers, and manage assigned member caseloads.

Required Qualifications

  • Active Licensed Vocational Nurse (LVN) or Licensed Baccalaureate Social Worker (LBSW) license.
  • 2 4 years of experience in care management, service coordination, case management, or LTSS.
  • Prior Service Coordinator (SC) or Care Manager (CM) experience preferred.
  • Strong organizational and time management skills.
  • Excellent communication and interpersonal abilities.
  • Proficiency with Microsoft Office and electronic documentation systems.
  • Ability to work independently while managing multiple priorities.

Preferred Qualifications

  • Previous experience in LTSS, Medicaid Managed Care, or Managed Care Organizations (MCOs).
  • Experience conducting home visits and community-based member assessments.
  • Knowledge of healthcare regulations, care coordination, and community resources.
