Clinical Liaison RN – Hospice


$10,000 Bonus   Job Type: Full-time with minimum on-call rotation




About the Role


We’re seeking a compassionate and motivated Clinical Liaison RN to join our hospice team. This role blends clinical expertise with relationship-building, helping patients and families understand hospice services and ensuring smooth transitions into care.




Key Responsibilities



  • Complete hospice eligibility assessments in hospitals, SNFs, and community settings

  • Educate patients, families, and referral partners on hospice philosophy and services

  • Coordinate admissions and ensure timely, accurate documentation

  • Build strong relationships with physicians, case managers, and discharge planners

  • Serve as a clinical resource to referral sources and internal teams



Requirements


 Qualifications



  • Active RN license in [State]

  • Hospice, palliative care, home health, or acute care experience preferred

  • Strong assessment and communication skills

  • Ability to work independently and in fast-paced environments

  • Compassionate, patient-centered approach
