Position Summary

The Therapist/Clinical Case Manager will provide individual therapy, case management, and clinical consultation as part of the Wake County UNC FIT Wellness Program, serving formerly incarcerated clients with serious mental illnesses. In addition to direct care, this role will support the development and supervision of the Community Health Workers (CHW) and contribute to administrative tasks that enhance program function.

Minimum Education and Experience Requirements

  • Master's degree in Social Work.
  • Must hold a LCSW license.
  • At least one year of direct experience with individuals with severe and persistent mental illnesses, particularly within justice-involved or high-need populations (for example, inpatient psychiatric care, assertive community treatment).
  • Experience providing therapy, clinical case management, substance use counseling, intervention and referral services to culturally diverse populations.
  • Strong engagement and rapport-building abilities, strong commitment to client-centered care, and experience and comfort working both independently and within a collaborative team.
  • Comfortable working with culturally diverse populations, LGBT individuals, and a diverse array of formerly incarcerated people including those who have committed a sex offense.
  • Valid NC driver's license and access to reliable transportation.

Preferred Qualifications, Competencies, and Experience

  • LCAS preferred.
  • Experience working with health care systems, mental health and substance use treatment programs, community-based organizations.
  • Experience with adult care home placement and residential substance use treatment referrals.
  • Experience with referrals for enhanced psychiatric services (e.g. ACT, CST).
  • Familiarity with interventions for co-occurring disorders and experience in working with a diverse client population.
  • Experience within the past 5 years working as part of a multi-disciplinary team, collaboratively practicing with peer support workers, psychiatrists, PCPs, health workers, health educators, and/or other health professionals.
  • Experience with Comprehensive Clinical Assessment (CCA), treatment planning, conducting interviews/investigations, advising/counseling clients, and participating in health promotion and health education activities.
  • Excellent organization, follow through, and ability to juggle multiple priorities in fast-paced environment with multiple collaborators
  • Excellent verbal and written communication skills.
  • Experience living or working in Wake County, NC
