Summary:
As a Clinical Hospital Pharmacist, you will be responsible for:
- Evaluating medication therapy for appropriateness, taking into account patient conditions, safety, efficacy, formulary status, and potential concerns related to drug-drug or drug-food interactions.
- Providing therapeutic drug level monitoring when appropriate.
- Effectively communicating with nurse/prescriber when errors are detected or to optimize therapy.
- Consulting with other practitioners as appropriate.
- Identifying trends during routine provision of pharmaceutical care, collecting data supporting concern, and formulating practice recommendations that improve outcomes or reduce cost.
Responsibilities:
- MS:
- Demonstrates accuracy when dispensing or preparing or mixing or labeling product or when filling automated dispensing machines or with computer data entry.
- Completes tasks with appropriate speed yet not sacrificing accuracy.
- Demonstrates reliability.
- Performs tasks in a conscientious, thorough and complete manner.
- Demonstrates a needs no reminder attitude after directions have been explained and carries out work assignments promptly.

- Identifies ways to decrease the possibility of medication dispensing errors.

Education and Experience:
- Generally requires 5 to 7 years of related experience.
- Pharm.D. degree and residency training or equivalent proven experience is required.
Requirements:
- Possession of or eligible for Florida pharmacist license is required.
- Ability to analyze, organize and prioritize work accurately while meeting multiple deadlines.
- Ability to communicate effectively in both oral and written form.
