Position Overview

CLFC Healthcare and Communications is seeking a licensed Clinical Case Manager for an outpatient substance use treatment program at a federal criminal justice supervision agency in Washington, DC. You are the connective tissue between the treatment program, the individual in recovery, and their Community Supervision Officer. When someone misses a session, you make the call. When someone needs housing, employment services, or mental health referrals, you build the bridge. When discharge approaches, you write the plan that determines whether that individual has a realistic path to sustained recovery.

This program serves adults on probation, parole, and supervised release who have been court-ordered into ASAM Level 1.5 outpatient substance use treatment or early intervention services. You will conduct intake interviews within seven business days of each referral, develop Individualized Care Plans, provide case management sessions addressing service coordination and crisis resolution, and prepare discharge plans with verified community resource referrals.

Key Responsibilities

  • Conduct intake interviews within seven business days of receiving each Billing Authorization
  • Develop person-centered Individualized Care Plans based on Brief Assessment results
  • Deliver case management sessions (30 min each, max 3 per individual) focused on service coordination, compliance, referrals, and crisis resolution
  • Re-engage individuals with unexcused absences through documented outreach within 24 hours
  • Prepare Non-Compliance Behavior Reports within 24 hours of each occurrence
  • Coordinate discharge planning staffings within 30 days of anticipated completion
  • Prepare comprehensive written discharge plans within 10 calendar days of discharge staffing
  • Maintain electronic treatment files meeting HIPAA, 42 C.F.R. Part 2, and DC DBH Chapter 63 standards
  • Submit monthly progress reports to the COR and CSOs

Required Qualifications

  • Master's degree in social work, psychology, counseling, or closely related field
  • Current licensure as LICSW, LCPC, LMFT, or CAC II in DC or eligible for transfer. Candidates with LGSW or LGPC under active supervision may also be considered.
  • 3+ years clinical experience in behavioral health services within the last 5 years
  • Experience developing treatment plans and coordinating multi-agency care for individuals with substance use disorders

Strongly Preferred

  • DC-specific licensure strongly preferred
  • Direct experience with criminal justice populations
  • Knowledge of DC community behavioral health resources, housing programs, and employment services
  • Experience with electronic health record systems, particularly DC HIE (CRISP)
  • Familiarity with ASAM Criteria
  • Bilingual capability (English/Spanish)
