Overview:
Scope of Responsibilities: Provide day-to-day support to clinical team and patient management program. Interact with health care professionals and patients to support clinical programs and initiatives.
Job Summary: Responsible for all aspects related to collecting, entering, and maintaining medical information within our electronic medical record platform. Participate in the collection and analysis of data for patient management and clinical programs. Responsible to ensure the highest quality healthcare delivery while complying with accreditation standards.

Job Duties:
Responsible for onboarding new and existing patients to our patient management program. Ensuring compliance with all accreditations and manufacturer programs and protocols
Assist leadership by supporting compliance with accreditation standards organizations such as URAC, ACHC, and NABP
Act as liaison with clinical, pharmacy, and call center teams in communicating with provider offices on questions regarding appropriateness of therapy, diagnosis, labs, or other questions/concerns
Responsible for appropriately documenting medical information into the patient record
Provides resolution to customer service issues to ensure patient and provider satisfaction
Participate in audit and quality review of activities performed by clinical department
Maintain and keep reasonable production as determined by supervisor
May assist in orienting and training new employees
Responsible for completing all mandatory and regulatory training programs
Perform other duties as assigned
Qualifications:
Educational Requirements:
Minimum: High School Diploma or GED
National Pharmacy Technician Certification through PTCB, or another accrediting agency
License/registration in good standing per State requirements
Experience:
Required: 3+ Years’ experience as a pharmacy technician
Special Conditions of Employment:
Drug test
Initial and continuous exclusion and sanction/disciplinary monitoring
Any and all additional eligibility requirements based on the specific position
Compensation
$19.00-22.00 per hour.
The final offer will be determined after careful consideration of multiple factors such as relevant skills, years of experience, and education.
