We provide quality resources in four specialty areas: Information Technology (IT), Clinical Research, Rehabilitation Therapy and Nursing.Job DescriptionJob Title: Clinical Care Certified Diabetes Educator (CDE)Location: San Antonio, TX Duration: 24+ months (Possibility of extension)Job Requirement:Candidates must be local.Certified Diabetes Educator Certification is requiredPosition Description:The Clinical Care Certified Diabetes Educator is part of the Integrated Care Solutions team and will have a positive attitude, outgoing personality, and a passion for helping patients succeed on our therapies.Each Clinical Care Certified Diabetes Educator will be a primary relationship manager and "coach" for a group of patients during the program duration and ensure that the patients stay motivated and successful throughout their journey.A strong predictor of success for the Clinical Care Certified Diabetes Educator is the ability to inspire our customer base, listen and understand the unique needs of every customer, communicate effectively on the phone and through chat and email, and execute strategies that efficiently engage and motivate the patient.Position Responsibilities:Drive higher engagement & retention rate of assigned group of new patients for the duration of the program through dynamic outbound calls at various touch points and inbound calls for help.Partner with the clinical team & other key stakeholders to meet individual needs of Health Care Providers (HCP) and patients and intervene with "at risk" patients when necessary.Assist patients with setting goals and realizing goals and identifying and overcoming concerns while enrolled in the program.Use dynamic communication skills to identify patient unmet needs and "pain points" and utilize objection handling techniques to communicate with the patient.Measure the success of the efforts through monitoring patient engagement, retention, patient learning metrics, quality scores and customer satisfaction ratings.Testing and conducting "proof of concept" sessions with patients to build web and mobile interactive solutions.Perform other duties as assigned.Basic Qualifications:Bachelor's degree from an accredited college/universityRN, RD, or Pharmacist license requiredCertified Diabetes Educator Certification is required3 years of clinical experienceDesired/Preferred Qualifications:Spanish fluencyKnowledge and understanding of Diabetes and/or Durable Medical EquipmentKnowledge of clinical & medical terminologyExperience in a case management based healthcare roleExcellent communication skills.Comfortable with technology, and quick to learn new technologies/solutions or potential ways to communicate with and contribute to our patient's success.Demonstrated ability to present information in a clear, concise, and grammatically correct manner both verbally and in writing.Demonstrated excellence in analysis, problem-solving, organization and administrative skills.Ability to effectively communicate with customers to resolve issues and answer questions according to department protocols.Proficiency in using a computer to navigate through multiple screens and programs at the same time in order to give and receive information.Ability to follow instructions and protocols when dealing with unfamiliar or unusual customer issues.Ability to generate reports, graphs, business correspondenceMust be able to work over eight (8) hours per day and weekends, if needed.Experience and proficiency in Automated Call Dial management and reporting2 years of experience working within Healthcare Diabetes
