Immediate need for a talented Care Navigator (Behavioral health). This is a 03 Months Contract opportunity with long-term potential and is located in FL (Remote). Key Responsibilities: Outreaching members, primary care physician and outpatient providers. Initiating appropriate assessment using our internal EMR system Creating a Care plan tailored to the member’s needs. Updating Medications, diagnosis, and allergies in our EMR system on a minimum of a monthly basis. Reviewing care gaps and offering education and resources in efforts to close those care gaps. Connecting members to providers and resources in the community. Addressing SDOH needs via community resources. Attending scheduled community staffing and treatment teams as well as partnering with community agencies.Key Requirements and Technology Experience: Must have skills: - ["Bachelor's", "Outreach", "Case Management", "Behavioral health"] Computer literacy Excel, Microsoft Teams, Outlook, OneNote Electronic Medical Record (EMR) Systems Requires a bachelor’s degree and 2 – 4 years of related experience. Requirement is Graduate from an Accredited School of Nursing if holding clinical licensure. Or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position. 2 – 4 years of related case manager or mental health experienceOur client is a leading Healthcare Industry and we are currently interviewing to fill this and other similar contract positions.
