A community services organization is seeking a Social Worker in Staten Island, NY, to conduct clinical assessments and support participants through a structured vocational rehabilitation program. This role focuses on comprehensive intake, documentation, and functional capacity evaluations for adults engaged in work-related services.

About the Opportunity:

  • Shift: Day shift
  • Schedule: Monday to Friday
  • Hours: 8am to 5pm or 8:30am to 5:30pm
  • Setting: Onsite

Responsibilities:

  • Orient participants to the program and guide them through the clinical assessment process
  • Collect, review, and scan supporting documentation to complete thorough assessments
  • Conduct participant interviews covering medical, behavioral health, substance use, housing, education, employment, and support history
  • Evaluate functional capacity and determine appropriate outcome recommendations based on clinical findings
  • Document services, assessment results, and participant interactions in internal management systems
  • Perform outreach to participants who miss appointments and carry out other related duties as assigned

Qualifications:

  • Active LMSW, LMHC, or LMFT license
  • Experience in mental health or behavioral health settings
  • Experience with assessments, intakes, and biopsychosocial evaluations
  • Proficiency with Microsoft Office and Teams
  • Ability to type 40 words per minute

Desired Skills:

  • Experience in substance use treatment
  • Experience working with individuals with intellectual and developmental disabilities
  • Bilingual Spanish language skills
  • Experience with vocational rehabilitation or disability-related assessments
  • Knowledge of functional capacity evaluations and sequential disability review
