Position Overview

We are seeking a detail-oriented Administrative Assistant to support our patient admissions process. This role is critical in managing pending admissions, tracking hospital discharges, and facilitating the smooth transition of patients to home health services. The Administrative Assistant will also prepare reports and serve as a key liaison between hospitals, intake, clinical leadership, and sales teams.

 

Key Responsibilities

Admission & Discharge Tracking

  • Manage pending admissions in WellSky EMR and track hospital discharge status daily.
  • Communicate with hospital case managers, referral sources, and intake teams to confirm discharge timing.
  • Initiate admission workflows and escalate barriers such as missing orders or authorization delays.
  • Conduct welcome calls and verify patient demographics, insurance, and physician information.

EMR Management & Documentation

  • Maintain accurate patient records and referral statuses in WellSky EMR.
  • Flag aging or stalled referrals and ensure consistent documentation to support reporting accuracy.

Reporting & Stand-Down Preparation

  • Prepare daily, weekly, and ad-hoc reports on pending admissions, discharge status, and referral trends.
  • Distribute stand-down reports to sales, intake, and leadership teams, providing clear patient-level updates.

Communication & Collaboration

  • Serve as a central communication point between intake, sales, clinical leadership, and referral sources.
  • Support smooth transitions for patients and participate in operational or intake meetings as needed.

Compliance & Quality Support

  • Adhere to agency policies, CMS regulations, and Conditions of Participation.
  • Maintain patient confidentiality and HIPAA compliance.
  • Support quality improvement initiatives through accurate and timely data management.

Requirements:

Qualifications

  • High school diploma or equivalent required; Associate’s degree preferred.
  • 1–2 years of experience in healthcare administration, admissions, intake, or scheduling.
  • Experience in home health, hospice, or hospital discharge planning strongly preferred.
  • Proficiency in WellSky EMR or similar systems required.
  • Strong skills in Microsoft Excel, Outlook, and reporting tools.
  • Excellent organizational skills, attention to detail, and ability to manage multiple priorities.
  • Strong written and verbal communication skills.
  • Analytical skills to interpret and present data clearly.
