Clinic Registered Nurse (RN) - Role Summary

The Clinic Registered Nurse (RN) provides high-quality, patient-centered nursing care within an outpatient clinic setting serving a frail and elderly population. Working under the direction of a Primary Care Provider and the supervision of a Clinic Supervisor, the RN supports daily clinic operations by managing clinical care, assisting with procedures, administering medications, and ensuring smooth coordination across care settings. This role is ideal for a compassionate, organized nurse who thrives in a collaborative environment and is committed to improving outcomes for vulnerable patients.

 Key Duties & Responsibilities

Clinical Care

  • Provide and document ordered nursing care and procedures in the clinic, including medication management and administration, in accordance with the scope of practice

  • Check vital signs, perform point-of-care testing, support wound care, and manage clinic overflow

  • Remain attentive to input from care team members, patients, and caregivers, promptly reporting changes in patient condition to appropriate clinical staff

  • Support care transitions between settings such as hospitals, skilled nursing facilities, home health, and clinic environments

  • Dispense ordered medications to patients during clinic visits and reinforce medication education

  • Perform and document ordered procedures during home visits when required, in accordance with the scope of practice

  • Provide patient, family, and caregiver education related to disease processes, self-care techniques, and prevention strategies

  • Perform pharmacogenomic (PGx) testing as directed by providers

  • Provide clinical support to Medical Assistants during clinic procedures

  • Process specialty pharmacy medication orders and escalate concerns to providers as needed

  • Address patient medication questions and concerns during clinic visits

  • Communicate medication incidents or errors during clinical briefings and ensure appropriate follow-up

  • Coordinate with providers regarding medication changes related to same-day procedures or external provider recommendations

  • Perform pre-enrollment medication evaluations

  • Provide coverage, as needed, in support roles such as Center Nurse, Medication Assistant, or Case Manager to ensure continuity of patient care

Administrative & Operational Support

  • Maintain clinical tracking logs, including TB screenings, influenza, pneumococcal, and shingles vaccinations

  • Monitor, restock, and document emergency cart supplies on a monthly basis

  • Stock medication dispensing systems and properly dispose of expired medications in accordance with policy, updating destruction logs as required

  • Provide backup coverage to other nursing functions as assigned to support clinic efficiency

Required Qualifications

  • Associate’s degree in Nursing from an accredited program

  • Active, unencumbered Registered Nurse (RN) license

  • Basic Life Support (BLS) certification

  • Minimum of one (1) year of nursing experience in a clinical setting with a frail or elderly population

  • Strong clinical skills to manage complex patient needs

  • Ability to work independently with minimal supervision

  • Excellent organizational, communication, and prioritization skills

  • Demonstrated ability to function effectively in a fast-paced, changing environment

  • Experience working with individuals from diverse cultural and socioeconomic backgrounds

  • Commitment to delivering compassionate care to vulnerable senior populations

Desired Qualifications

  • Bilingual English/Spanish proficiency (preferred)

  • Experience in outpatient clinic or primary care settings

  • Strong teamwork and care coordination skills

Location & Work Type

  • Work Type: On-site, clinic-based

  • Schedule: Monday–Friday, 8:00 a.m. – 5:30 p.m.
