Qualifications:


RN licensed in Idaho

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1.    Provides medical care in compliance with the state Nurse Practice Act under the supervision of a collaborating provider.

2.    Maintain performance and productivity standards, operational efficiency, excellentcustomer 
service, and best practice standards.  

3.   Treat acute and routine medical conditions via in person, telephone call, or telehealth across all 
settings (e.g. Clinic, SNF, ALF house call, visits), 

4.   Demonstrate critical thinking, expert care, and best practice standards in the clinical decision-
making process. 

5.   Provide care for patients from initial history taking,clinical assessment, diagnosis, providing education, and evaluation of care. 

6.   Maintain compliance with the state Nurse Practice Act.

7.   Document findings and actions appropriately in the patient’s medical record. 

8.   Maintain patientconfidentiality at all times. 

9.   Participate in professional development activities and maintain professional affiliations and CME’s. 

10. Maintain safe work environment for self, patient, family and staff by using appropriate PPE. 

11. Work collaboratively with Horizon team members and community partners such as Hospitals, ALF 
and SNF staff members, patient family members, and home health and hospice team members. 

12. Represent Horizon in a professional manner and strive to promote a cooperative atmosphere at all times by acting with tact and diplomacy and maintaining a positive attitude. 

13. Always maintain professionalism by being dependable, autonomous and responsible.   
14. Prioritize workload and utilize work time properly and productively. 

15. Be humble and possess the ability to accept criticism and/or discipline
