Matrix Medical Network seeks a Nurse Practitioner to deliver high-quality in-home and community-based health assessments for diverse adult populations. In this role, you will perform comprehensive exams, review medical histories, manage chronic conditions, and develop evidence-based care plans that enhance patient outcomes and support value-based care. You’ll collaborate closely with an interdisciplinary team, leverage telehealth and digital tools, and ensure accurate EHR documentation and regulatory compliance.
