What You'll Do Patient Care & Clinical Support

Conduct patient assessments, triage, screenings, and care coordination in collaboration with providers Administer medications, treatments, immunizations, and injections per provider orders Assist with exams and procedures; perform wound care, specimen collection, and lab testing Monitor patient responses and report critical values promptly Respond to medical emergencies and provide appropriate interventions

Care Coordination & Education

Communicate with patients, families, providers, and care teams to ensure continuity of care Educate patients and families on health conditions, treatment plans, preventative care, and wellness Identify health literacy needs and provide appropriate education and materials

Documentation & Quality

Maintain accurate, timely documentation in the electronic medical record Support audits, quality improvement initiatives, and regulatory requirements (PCMH, UDS) Participate in performance optimization and risk management activities

Team & Clinic Support

Perform medical assistant duties when needed and maintain competence Model One Health's mission, values, and commitment to exceptional service

What We're Looking For Required Qualifications

Nursing degree from an accredited nursing program Current nursing license in the state of Montana BLS certification (or ability to obtain upon hire) Valid Driver's License

Preferred Qualifications

Registered Nurse (RN) Two (2) years of nursing experience Experience in a medical clinic or primary care setting Experience working with diverse populations
