The Client Care Manager serves as a key leader within our organization, ensuring exceptional client experiences through non-medical care coordination, caregiver supervision, quality assurance, and ongoing client support.

This position works closely with clients, families, caregivers, hospitals, skilled nursing facilities, assisted living communities, and referral partners to promote continuity of care and outstanding service outcomes.

Peggy's Home Care is a non-medical home care organization. This role is administrative, supervisory, educational, and care-coordination focused and does not provide skilled nursing services.

What You'll Do

  • Conduct non-clinical client assessments and establish service plans
  • Perform supervisory and quality assurance visits
  • Coordinate services for clients and caregivers
  • Coach, mentor, and train caregivers
  • Support client transitions from hospitals, rehabilitation centers, and assisted living communities
  • Collaborate with discharge planners, case managers, social workers, and facility staff
  • Investigate concerns and resolve service issues
  • Review documentation and identify opportunities for improvement
  • Support infection prevention, safety, and compliance initiatives
  • Assist with leadership and oversight of Care Coordination and Staffing teams
  • Build and maintain strong referral source relationships 
Qualifications

  • RN, LVN, Social Worker, or healthcare professional with comparable leadership and care coordination experience
  • 3–5 years of experience in home care, healthcare operations, hospice, home health, case management, caregiver supervision, or senior services preferred
  • Strong communication and relationship-building skills
  • Excellent organizational and problem-solving abilities
  • Proficiency with Microsoft Office and technology platforms
  • Ability to work independently while managing multiple priorities
  • Valid driver's license and reliable transportation
