MD or DO from an accredited institution, board-certified in Family Medicine or Internal Medicine. Minimum 3 years of provider management experience; FQHC experience highly preferred. Proven ability to lead multi-disciplinary teams and navigate complex healthcare environments. Strong understanding of value-based care, patient-centered medical home (PCMH) principles, and quality improvement frameworks including UDS and HEDIS. Ability to balance strategic leadership with hands-on clinical engagement.
