Certified Occupational Therapy Assistant (COTA) - Home Health

We are seeking a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our Home Care team and help patients regain function, improve safety, and maintain independence following illness, injury, or surgery.

As a COTA with Cornerstone VNA, you'll have the opportunity to build meaningful relationships with patients while providing one-on-one care in the home setting. You'll work alongside experienced therapists and an interdisciplinary healthcare team dedicated to helping patients achieve their highest level of function and quality of life. What You'll Do

  • Provide occupational therapy treatments under the direction and supervision of a licensed Occupational Therapist.
  • Implement individualized treatment plans designed to improve functional independence and safety.
  • Assist patients in developing or restoring skills needed for daily living activities.
  • Educate patients and caregivers on adaptive techniques, equipment, and home safety strategies.
  • Monitor patient progress and communicate changes to the supervising therapist.
  • Complete timely and accurate documentation within the electronic medical record.
  • Coordinate care with therapists, nurses, physicians, and other healthcare professionals.
  • Manage a daily schedule of patient visits while maintaining excellent patient care and service.
  • Participate in ongoing education, training, and quality improvement initiatives.

What We're Looking For

  • Graduate of an accredited Occupational Therapy Assistant program.
  • Current Occupational Therapy Assistant license in New Hampshire and Maine.
  • Certified Occupational Therapy Assistant (COTA) credential required.
  • Minimum of one year of home health experience preferred.
  • Minimum of two years of therapy experience preferred.
  • Strong clinical, communication, and organizational skills.
  • Ability to work independently while collaborating effectively with a multidisciplinary team.
  • Reliable transportation and ability to travel throughout the service area.
