Provide compassionate, resident-centered care while assisting with activities of daily living, including bathing, dressing, grooming, toileting, feeding, oral hygiene, and mobility.
Assist residents with repositioning, transfers, and ambulation while promoting comfort, dignity, and independence.
Obtain and accurately document vital signs, weights, intake and output, and other assigned clinical observations.
Collect routine specimens as directed.
Perform restorative nursing activities in accordance with resident care plans.
Observe residents for changes in physical or emotional condition and promptly report concerns to the licensed nursing staff.
Respond promptly to call lights and resident requests.
Assist with admissions, transfers, and discharges.
Maintain clean, organized, and safe resident rooms and common areas.
Complete rounding assignments, including safety and comfort checks.
Accurately document care provided in the electronic medical record.
Communicate effectively with residents, families, visitors, and members of the interdisciplinary healthcare team.
Follow all infection prevention, safety, and resident rights policies.
Current Pennsylvania Certified Nursing Assistant (CNA) certification required, or eligibility to work in Pennsylvania with an Ohio or West Virginia credential.
Current CPR certification preferred.
Previous long-term care, skilled nursing, rehabilitation, or hospital experience is preferred but not required.
