**Job Summary**
Augments the service of the licensed nurse by providing basic patient
care to the hospice patient.
**Essential Job Functions**
- Assists in the care of the hospice patient including activities of
daily living (ADL\'s) such as, but not limited to, personal hygiene
and grooming, bathing, feeding, repositioning, toileting, transfers
and ambulation; household services essential to promote a clean,
safe environment.
- Accountable for reporting and recording tasks performed as well as
any noticed change in the patient\'s condition.
- Must be able to relate well/work closely with patients, their
families and coworkers.
- Obtain and record patient data such as, but not limited to, vital
signs, catheter care, pain levels, report findings outside
parameters as noted on the assignment sheet or urgent/emergent
conditions. Completes appropriate records as required.
- Participate in interdisciplinary team meetings to discuss and
coordinate the patient plan of care.
- Requires creativity and ability to problem solve in the home
environment.
- Participate in required staff development activities and performance
reviews.
- Attendance at meetings required, indoors 90% of the time.
- Position requires access to highly confidential information.
- Performs other duties as assigned.
**Minimum Requirements:**
- High school graduate preferred.
- Completion of state approved nursing assistant training
program/current Board of Registry status required.
- Completed and cleared Idaho state background check.
- Current BLS certification.
- A minimum of 12 hours of hospice-specific training annually is
required to continue employment in hospice department.
**Skills/Competencies**
- Able to demonstrate the ability to prioritize tasks, use good
judgement in the delivery of patient care and communicate
effectively with patients, family members, care team members and
providers.
