Certified Medical Assistant - Clinical Pharmacy Team

Job Summary

The Certified Medical Assistant will function in conjunction with a clinical pharmacist to deliver care within a Federally Qualified Health Center. Duties include performing as a member of the team and assisting in the administration and maintenance of an efficiently operated clinic. This role is essential in promoting preventive care, addressing health disparities, and improving outcomes for patients. The Certified Medical Assistant must demonstrate a sincere dedication and loyalty to the mission, vision and core values of BRMC. Clinical Pharmacy visits currently consist of Medicare Annual Wellness Visits, but may be expanded to include acute visits or focused clinics. This position may involve assignments at satellite locations as needed.

Essential Job Duties

1. Conduct outreach to schedule Medicare Annual Wellness visits, prioritizing high-risk patients by using appropriate portals and insurance information.
2. Conduct pre-visit planning for efficient visits.
3. Greet and room patients ensuring two patient identifiers.
4. Obtain vital signs, weight and height and any other pertinent data per center protocols.
5. Prepare treatment rooms for visits with patients.
6. Perform medication reconciliation.
7. Administer and document standardized screening tools (depression, fall risk, cognitive assessment, social determinants of health)
8. Assist in examination and treatment of patients. Perform necessary test(s) as directed by provider. [KB1]
9. Administer immunizations under the direction of a provider.
10. Collect, label, and prepare specimens for lab. [KB2]
11. Document patient care provided in the patients’ electronic health record. Document all quality measures accurately in structured EHR fields.
12. Support coding accuracy and quality reporting requirements.
13. Assist with routine maintenance activities including, but not limited to, the following: clean equipment, stock exam rooms, organize clinical work area, etc.
14. Participate in value-based care and clinical quality improvement activities in accordance with applicable standards.
15. Complete work activities successfully through effective management of time, tasks and available resources. Remain vigilant in the performance of duties.
16. Perform other necessary duties as assigned by the Clinical Pharmacist to meet the goal of providing quality health care services.
