In this role, you will provide essential personal care and support to patients while being a valued member of the interdisciplinary care team.

What You’ll Do

As a Home Health Aide, you will provide hands-on support to patients in their homes, helping ensure comfort, safety, and dignity. Responsibilities include:

  • Assisting with personal care needs such as bathing, grooming, dressing, and toileting
  • Supporting skin care and hygiene to prevent breakdown
  • Preparing meals and assisting with feeding when needed
  • Performing light housekeeping and maintaining a clean, safe environment
  • Assisting with mobility, ambulation, and range-of-motion exercises
  • Taking and recording vital signs as directed and within scope
  • Assisting with self-administration of medications (as allowed)
  • Observing and reporting changes in patient condition to the care team
  • Documenting care and maintaining accurate records
  • Participating in care team meetings and case conferences
  • Supporting patient safety and proper use of equipment
  • Participating in on-call, weekend, and holiday rotations

What We’re Looking For

  • Completion of a state-approved Home Health Aide training and competency program
  • Listed in good standing on the state nurse registry
  • Minimum age of 18 years
  • Ability to follow written care plans and document services provided
  • Ability to work independently with minimal supervision
  • Compassion, patience, and strong interpersonal skills
  • Commitment to maintaining patient dignity and confidentiality
  • Valid driver’s license with a reliable, insured vehicle
