Job Description

GENERAL OVERVIEW Assesses patient need for diabetes education. Provides individualized diabetes self-management education and training based on assessment. Plans and conducts continuing education programs for professional staff and the community.

ESSENTIAL RESPONSIBILITIES • Collects DSMET program participant assessment data using a recognized certification database program in a collaborative and ongoing manner. • Develops educational goals, learning objectives, and a plan for educational content and teaching methods with program participants. • Collaboratively develops an individualized follow-up plan with each program participant. • Evaluates effectiveness of educational services provided by measuring attainment of learning objectives. • Conducts a follow-up assessment upon completion of DSMET program services using outcome measures from the AADE7 Continuum of Outcomes Framework. • Documents assessment data, educational plan, educational services provided, and evaluation results in each participants educational records. • Participates in the development of education curricula and materials. • Performs other duties as assigned or required.

QUALIFICATIONS

Minimum • Bachelors Degree • Registered Licensed Dietitian • Certified Diabetes Care and Education Specialist (CDCES) or eligible and must obtain CDCES certification within nine months of hire • Experience in counseling patients with diabetes and experience in community education • Act 34 Criminal Background Clearance Certificate • Act 33 Child Abuse Clearance Certificate • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate

Preferred • Masters Degree • 1-3 years experience in Diabetes Education and Health Living Coaching • Certified insulin pump trainer (CPT)
