- Provide case management services for children ages 11 - 21 with mental health and/or co-occurring disorders as part of the Children's Community Action Treatment (CAT) Team in Charlotte County.
- Provide services in the community; including school, home, etc.
- Interact with community partners and other stakeholders, as appropriate.
- Hours include evenings and on-call, as necessary or assigned.
- Conduct assessments, create treatment plans, coordinate and provide treatment and supportive services to children and their families.
- Provide appropriate referrals for additional services and resources, as identified.
- Provide support and advocate for children and family's rights and needs.
- Assist with development of natural support system(s).

- Bachelor's degree in human services field.
- Minimum one (1) year experience working with children and families.
- Knowledge and understanding of the characteristics of children with mental health concerns, families, and family systems preferred.
- Ability to perform repetitive tasks.
- Ability to sit or stand for extended periods.
- Ability to safely operate a motor vehicle in all driving conditions.
- Ability to lift up to 10 pounds.
- Ability to pass a level II Background clearance and drug test.
- Valid FL drivers' license, insurance, and safe driving record.
- Dependable transportation (registered, safe operating conditions, etc.).
- Must maintain high standards of ethical and professional conduct, while adhering to agency policies and procedures.
- Ability to use a computer.
- Attention to detail.
- Ability to work independently and as part of a team, in collaboration with other community partners.
- Ability to manage stressful situations and display appropriate work demeanor and boundaries.
- Ability to demonstrate excellent customer service.
- Strong oral and written communication skills.
