Caseworker Specialist will need to be able to:Serve as the initial point of contact for guardianship inquiries by screening calls and email referrals, collecting key information, and determining eligibility and priority level.Assess current and potential client/patient needs and also develop and implement plans to meet those needs.Monitor client funds, which includes managing client financial accounts or budget activities and providing fiscal information, and preparation of annual court accounting reports.Apply for and maintain public benefits eligibility.Prepare detailed case documentation including opening and closing cases, intake records, investigation summaries, case histories, activity logs, risk/needs assessments, social summaries, service plans, reports to the court, and correspondence, etc.Provide clients with information concerning rights, options, benefits, services, goals and expectations.Conduct monthly visits with clients and complete documentation.Collaborate and coordinate with service and medical providers.Perform field visits and determine the need for guardianship and/or health and well-being of an adult.Coordinate with and refer to community resources, placement providers, collateral agencies, the court, and/or other community partners for clients receiving services.
