Case Manager - Day Habilitation

SUMMARY: The case manager assumes primary responsibility for each member on an assigned caseload and coordinates the implementation of the program Service Plan and maintains each member’s record.

ESSENTIAL FUNCTIONS:

  • Develop and ensure the implementation of the program Service Plan.
  • Serve as liaison between the program and each member’s support system.
  • Complete and submit all assessments as required.
  • Maintain and update all service records.
  • Maintain an expertise in the field through attendance at meetings, trainings, continuing education or other
  • appropriate educational programs.
  • Attend interdisciplinary team meetings as needed to participate in curriculum design.

QUALIFICATIONS:

  • High school diploma or equivalent
  • At least one-year case management experience in a program for developmentally disabled adults, or three years’ experience in Human Services or a related field.
  • Skill in both written and oral communication.
  • Ability to working both independently and as a team member.
  • Ability to effectively participate in the design, implementation, and coordination of services.
  • Ability to complete and disseminate all required forms and documents.
  • A qualifying criminal background check, driving record, and a valid driver’s license required.
