The Case Manager – Specialized, serves as the link between the client and care delivery system through the coordination of community-based services to provide access to quality mental health and program service needs. The role of a Case Manager – Specialized is to coordinate and provide care that is safe, timely, effective, efficient, equitable, and client centered. They project manage key initiatives, facilitate case assignments, draft service plans, monitor case progress, and monitor service contracts. The Case Manager helps clients achieve wellness and autonomy. The ideal candidate is someone who uses their compassion and resourcefulness to bring positive change to their clients’ lives. C.O.P.E. case management oversight of services includes working with children, youth, and young adults over an array of service areas included but not limited to parent education, anger management, Student Academic Review Board (SARB), youth achievement programs, coaching/mentoring, and counseling-based services. The Case Manager – Specialized is a part of the C.O.P.E. case management series and is distinguished from the entry level Case Manager position as this position has advanced responsibility for project management (planning and implementation) and project monitoring.

ESSENTIAL DUTIES & RESPONSIBILITIES:

The following breakdown is intended to provide a general idea of how the Program Coordinator will spend their time, but this list should not be considered an exact representation of duties:

  1. 50% of time: Project Management
  2. 25% of time: Case Management
  3. 25% of time: Administrative
