Position Summary:
Collaborates with medical staff, unit managers, and all professional care giver disciplines to plan and coordinate the plan of care of patients and families throughout the episode of illness and across the continuum of care settings, utilize resources to maximize the health care benefits to patients in a cost-effective manner. Participates in the utilization review process and assists in guiding quality care in accordance with best practice guidelines and protocols by communicating effectively with physicians. Responds to team leader and management as required.
Position Qualification:
BSN required; if ASN must be willing to attain BSN. Must be a registered nurse in the state of Tennessee. Case Management certification preferred.
