An acute care hospital in the Bronx, NY area is seeking a licensed Registered Nurse (RN) to join their Inpatient team as a Care Manager. In this role, the Case Manager (RN) will support patient care coordination and discharge planning. This role focuses on utilization review, interdisciplinary collaboration, and helping patients transition safely through the continuum of care.
About the Opportunity:
Shift: Day shift
Schedule: Monday to Friday
Hours: 9am to 5pm
Setting: Acute care hospital
Responsibilities:
- Coordinate inpatient case management activities from admission through discharge.
- Conduct utilization review and support PRI and SCREEN processes.
- Develop, monitor, and update individualized care plans based on patient needs.
- Collaborate with patients, families, physicians, nurses, insurance providers, and interdisciplinary teams.
- Identify community resources, facilitate referrals, and maintain accurate documentation of care plans and progress.
Qualifications:
- Active New York State Registered Nurse license.
- Minimum of 5 years of clinical nursing experience.
- Experience in care coordination, utilization review, discharge planning, and computer literacy.
- PRI and SCREEN certification required at time of submission.
- AHA BLS certification required.
- Associate degree in nursing from an accredited program.
Desired Skills:
- Experience with the EPIC EMR system.
- Bachelor's degree in nursing from an accredited program.
