A Case Manager RN opportunity is available at a 267-bed hospital in Fort Walton Beach, FL, offering an Advanced Primary Stroke Center, a Cancer Care Center, open-heart surgery services, and a main emergency room with a dedicated pediatric ER and two freestanding ERs.
The role. In this position, you will support patients and families through every step of the care journey by coordinating services, connecting resources, and developing individualized care plans that reflect each patient's unique strengths, needs, and recovery goals. You will guide patients and families through program orientation, covering rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights. A core part of the role involves completing psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team. You will also coordinate education for patients, families, and caregivers to encourage active participation in treatment planning, goal discussions, and family conferences. Throughout, you will collaborate closely with the Rehab Program Director, Facility Case Management Director, and an interdisciplinary team to ensure seamless, patient-centered transitions across the continuum of care.
What we're looking for.
• Associate Degree or Certificate
• Registered Nurse (RN) license
• 2 years of recent (within the last 3 years) acute care experience; ICU, CCU, Med-Surg, or LTAC Case Management experience preferred
Schedule. Days with rotating weekends, 8:00 AM to 4:30 PM.
Compensation. $34.25 - $47.10/hr plus Sign on: case by case NOT guaranteed
