- Full-Time
- Day Shift: 8:00 AM – 4:30 PM
- 1 Weekend Every 4 Weeks
Compensation
- $31.94/hr – $43.92/hr (based on experience)
Additional Compensation
- Evening Differential: +$2.50/hr
- Weekend Differential: +$2.00/hr
- Shift differentials may stack when applicable

Position Overview
The Case Manager RN is responsible for coordinating patient care and discharge planning within an acute care hospital setting. This role focuses on care coordination, utilization review, patient advocacy, and ensuring safe transitions of care while supporting quality patient outcomes.
Key Responsibilities
Care Coordination & Discharge Planning
- Perform patient assessments and develop individualized care plans
- Coordinate interdisciplinary care and discharge planning
- Facilitate patient transitions across the continuum of care
- Identify and address barriers to discharge and patient throughput
- Coordinate post-acute services and community resources
Utilization Review
- Evaluate admissions and ongoing stays for medical necessity
- Review level of care and admission status appropriateness
- Communicate with payors regarding utilization management
- Monitor variances and support process improvement initiatives
Patient Advocacy
- Serve as liaison between patients, families, providers, and care teams
- Educate patients and families regarding discharge and care resources
- Support high-risk and under-resourced patient populations
Quality & Compliance
- Maintain accurate documentation in the medical record
- Support patient safety and quality improvement initiatives
- Ensure compliance with hospital policies and regulatory standards
Required Qualifications
- Associate Degree in Nursing or Nursing Diploma required
- Active RN License required
- Minimum 3 years of recent acute care or case management experience required
Acute Care Backgrounds Considered
- Med/Surg

Preferred Qualifications
- Bachelor’s Degree in Nursing preferred
- Case Management or Utilization Review certification preferred
- Strong discharge planning and interdisciplinary collaboration skills
