Job Summary and Qualifications Under the supervision of the Nurse Manager, the Case Manager performs the primary function of an RN in assessing, planning, implementing and evaluating the care of patients in the department. What you will do in this role: You will utilize good problem solving abilities, making sure that problems are resolved, calling on other knowledgeable parties as necessary. You will evaluate collected data and prepare reports assessing accuracy, completeness, timeliness, progress, adverse trends and appropriate recommendations or conclusions. You will maintain confidentiality by following hospital rules and reminding others when necessary. (HIPPA) You will actively participate in continuous quality improvement activities by making suggestions for improvement and helping to make positive changes to include quality control, quality assurance, safety and inspection control practices. You will actively participate in the team spirit of the department, helping to accomplish the department mission, and lending a helping hand to other members of the team as appropriate. What qualifications you will need: Graduate of an Accredited RN program Registered Nurse (RN) license required or RN Compact State Licensure in the State of Residence Case Management Certification (CCM) or American Case Management Association (ACMA) certification preferred 3-5 years acute care experience and Case Management or Utilization Management preferred Benefits Parkridge Medical Center, offers a total rewards package that supports the health, life, career and retirement of our colleagues.
