The Road Home was founded in 1923 and has been a leader in ending homelessness for over 100 years. We provide emergency shelters, supportive services, and housing-focused-minded rental assistance that helps individuals and families step out of homelessness and back into the community. The Permanent Supportive Housing Case Manager provides trauma-informed, family-driven case management services to unhoused families as they transition from homelessness into stable housing throughout Salt Lake County. This role works closely with each family to assess their unique goals, identify needs, and address barriers to self-sufficiency. The Case Manager connects families to vital community resources and partners with them to develop personalized plans that support long-term housing stability and overall well-being. This position requires a deep commitment to serving families with compassion, respect, and a strengths-based approach that empowers them to thrive in their new homes.
