Registered Nurse - Case ManagerLocationLake & Marion County FL Ocala areaScheduleMonday-Friday800 AM - 500 PMTravel RequiredBetween multiple skilled nursing facilitiesDress CodeMedical attire scrub color not specifiedPrimary ResponsibilitiesEnsure accurate and complete documentation in compliance with clinical and regulatory standards.Collaborate with the interdisciplinary care team to support patient care and coordination.Assist providers with care coordination activities including benefit determination for Medicare Part A B and D.Partner with the primary Advanced Practice Clinician APC and facility staff to identify and respond to changes in patient condition.Support therapy coordination and participate in facility-based meetings and family conferences.Coordinate training track compliance and support quality measure initiatives.Conduct and assist with in-service training for selected audiences.Aid providers in documentation coding medication reconciliation nursing rounds and assessments.May write verbal orders from providers in compliance with state RN practice laws and facility protocols.Assist in managing patients during acute medical episodes or short-term sub-acute rehabilitation. Required QualificationsActive and unrestricted RN license in the state of residence or ability to obtain by start date.Associate degree or higher in Nursing.Certified in Basic Life Support BLS.Ability to lift up to 30 pounds and perform physical assessments.Access to reliable transportation.Fully vaccinated against COVID-19 compliance with federal state and client requirements.Preferred QualificationsMinimum 1 year of clinical experience long-term care setting preferred.Experience with patients who have complex behavioral social or functional needs.Understanding of geriatrics and chronic illness management.Proficient in electronic medical records and documentation.Strong communication and time management skills.Ability to work with diverse care teams across multiple facilities.Willingness and ability to travel between facilities during shift hours.
