A nonprofit services organization in New York City is seeking a Licensed Practical Nurse (LPN) to join their Preventive Services Program as a Case Manager.

About the Opportunity:

Schedule: Full-time
Hours: 9am to 5pm or10am to 6pm or 11am to 7pm

Responsibilities:

Conducting family-based home visits
Assessing health concerns and providing education/support
Helping families understand medical recommendations and follow-up care
Serving as a liaison between families, hospitals, clinics, and community providers
Coordinating care with social workers, program leadership, and medical providers
Supporting preventive health, chronic illness management, maternal/child health, and family health education
Completing documentation and follow-up as needed

Qualifications:

Graduate of an accredited School of Practical Nursing
Active NYS Licensed Practical Nurse (LPN)
Solid assessment, clinical, and documentation skills
