Are You Ready to Make a Difference?
We are currently seeking a Case Manager in the role of Independent Living Coach. The successful candidate will assist participants to meet program and operational objectives, determine services and housing eligibility needs based on Treatment/Life Plan implementation. The key responsibilities of this role include:
• Bachelors Degree in human services • 1 years experience working with adult population • Organizational skills and ability to develop Treatment/Life Plans that are easily followed, measurable and attainable • Possess initiative and the ability to work effectively without close daily supervision

Hourly Pay Rate
The hourly pay rate for this position is $19.78 to $20.77 based on the highest level of education.
