Salary Range: $23.19 – $24.90 per hour

The Case Manager I provides comprehensive case management services to individuals with Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), substance use disorders, and co-occurring conditions. This position supports recovery, engagement, life-skills development, resource coordination, and community integration.
• Provide case management and linkage services to unhoused and vulnerable individuals
• Coordinate care plans and connect clients with community resources
• Participate in court hearings and collaborate with justice system partners
• Monitor client progress and communicate with treatment team members
• Assist with daily living skills, medication education, and recovery support
• Maintain timely and accurate documentation in SmartCare EHR
• Assist with program implementation and service delivery activities
• Perform other duties as assigned
• High School Diploma or equivalent
• One (1) year of related experience in healthcare, social services, or behavioral health
• Valid California Driver’s License
• Knowledge of community resources and social service systems
• Strong communication and relationship-building skills
• College coursework in a related field
• Bilingual English/Spanish preferred
• CNA or Medical Assistant experience desirable
• Lived experience supporting individuals in recovery valued
