Openwork is hiring an experienced Case Manager / Housing Facilitator to support young adults (ages 18–24) as they work toward stable housing and independent living. This is a rewarding opportunity to make a meaningful impact by helping individuals access housing, community resources, and ongoing support.
What You'll Do:
Assist clients with housing searches, lease applications, and move-ins.
Help participants with housing vouchers and required documentation.
Conduct assessments and develop individualized service plans.
Provide ongoing case management and support.
Maintain accurate case notes and client documentation.
Connect clients with community resources and partner agencies.
Build relationships with landlords and housing providers.
Participate in program activities and client engagement efforts.
What We're Looking For:
2+ years of case management experience required.
Experience with housing programs, supportive housing, housing navigation, or housing vouchers required.
Experience working with young adults or social service programs preferred.
Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Criminal Justice, or a related field required.
Strong communication and organizational skills.
Ability to manage multiple cases and maintain accurate documentation.
Must be available to work evenings and weekends as scheduled.
