    SummaryUnder general direction, the Case Manager will coordinate existing services for the unhoused in areas such as prevention, early intervention, and support services. Establishes, maintains, and enhances cooperative relationships with local service providers and the unhoused community. Evaluates the social needs of each client and formulates a written plan of care. Refers clients to appropriate community resources as necessary.Essential Duties And Responsibilities

    Minimum Qualifications

    Combination of education and experience equivalent to a Bachelor's degree in social work, public health, or related areaTwo (2) years of experience in the field of social services with unhoused or low-income services or related case management area

    Preferred Qualifications

    Experience working with diverse agencies and community groups in a local government or non-profit agencyExperience in Community outreach, advocacy, and public education

    Certificates, Licenses, Registrations

    Valid driver's license
