Job Title: Case Manager HPMC Case Manager: 2 years of Case Manager experience required. RN License required, nursys must be on profile. BLS required. Degree accreditation or 5 years of experience required. *** clinical prescreen with Manager having final interview and offer.
Radius Rule 50 miles from perm address. FLOATING REQUIRED, Travelers are first to float, possibly float to other sister campuses as well. ALL RTO must be on submission, HCF will not be approving RTO after submission. (Please load all Demographic information at time of submittal: Address, email, full DOB, full SS, RN license number.)
