Essential Responsibilities
Coordinate outreach efforts to engage parents and other potential team members in case management care coordination services. Identify immediate and/or potential crises and safety concerns and develop a plan that addresses these concerns and prevents future crises.
Develop a comprehensive family care plan with measurable objectives and action steps that focus on strengths, needs, and culture, and include team involvement in helping parents achieve their goals. Link parents with services and ensure the integration of health care across adult and child behavioral health systems by educating parents on their parental rights.
Support and utilize existing networks and coalitions within the community to enhance communication and increase collaboration in service planning and delivery of services. Travel required between offices and other sites, including homes, schools, and community locations where clients are served.
Requirements
Bachelors degree in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related field that includes the study of human behavior and development.
Minimum of one year of experience working with adults with behavioral health needs and child welfare involvement.
Bachelors or Masters degree with a major in another field and a minimum of three years of experience working with adults with substance use disorders.
Must have a valid Florida drivers license and applicable state required car insurance.
Salary Rate
$22.63 per hour
