In this role, you'll coordinate and monitor individualized care plans that help older adults maintain their independence and quality of life. You'll collaborate closely with clients, caregivers, families, and community partners to ensure supportive services-such as caregiving, homemaker, and chore assistance-are delivered with compassion, efficiency, and respect.
