General Description
Provides moderately complex (journey-level) case management work. Work involves developing and maintaining long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs. Will be responsible for assessing and linking youth and families to community based providers for services. May supervise the work of others; Works under general supervision with moderate latitude for the use of initiative and independent judgment.
Examples of Work Performed
Will provide service coordination to all youth and families.
Will coordinate service referrals, monitor youth/family engagement in referred services.
Assist with program networking and collaboration with outside agencies to enhance the service delivery for the juvenile and families.
Refers youth and family to community based providers and monitors service engagement which will aid in promoting long term success, reduce substance abuse, increase pro social activities, increase family communication/interaction and reduce recidivism by helping the family stabilize their living environment.
Group facilitator or job readiness classes, life skills and in-home and group provider of parenting of classes.
Demonstrates respect and dignity towards team members, youth and families at all times.
Assists client and/or their family in assessing their psychosocial and/or resource needs assessment and obtains maximum benefit from all available sources.
Provides the youth and family with support services to prevent juvenile delinquency.
Provides supportive skill building to youth and families.
May act as an interpreter and must be able to translate both spoken word and written texts to the target language with superior accuracy, consistency and efficiency.
Responsible for management of grant.
Identifies problem areas, service gaps, and areas that are barriers to services for clients.
Makes recommendations on policy and procedure changes.
Interviews clients or authorized representatives to gather information to assess service needs.
Develops and implements service plans to meet client needs.
Documents case records.
Promotes progress and assists clients toward self-sufficiency, health and well being.
Ensures full professional accountability.
Participates in case reviews involving client/family and other service providers.
Establishes and maintains effective working relationships with clients and other social service and/or governmental agencies.
Acts as a liaison between all the parties involved (which may include attorneys, health organizations, community agencies and/or other organizations).
Employee may be assigned other duties in addition to those listed; duties may change according to the changing needs of the County.
May perform other duties related to the grant.
Education and Experience
Graduation from an accredited college or university with a Bachelor's degree in Social Work, Psychology, Criminal Justice or related field.
One (1) year of experience in social work counseling at-risk youth.
Certificates, Licenses, and Registration:
Must have a current valid Texas motor vehicle operator's license.
Must be able to be insured by the County's insurance carrier.
Knowledge, Skills and Abilities
Knowledge of Texas laws, codes, and standards related to the area of responsibility.
Knowledge of community resources; of case management principles, objectives, standards, and methods; and of program policies and procedures.
Knowledge of effective group facilitation skills and ability to facilitate adolescent group/parenting counseling sessions.
Must have planning and organizational skills.
Knowledge of service providers in the community whom are able to assist youth and families meet their identified needs by providing resources that will promote long term success and reduce recidivism by helping families stabilize their living environment.
Skill in conducting inspections and interviews, in determining proper courses of action, and knowledge in using techniques, guidelines and standards to perform inspections.
Ability to prepare reports, interpret laws and regulations.
Proficient in the use of various software computer applications; Microsoft Word, Excel, Power Point.
Bilingual (Spanish and English) with the ability to converse fluently in both languages.
