Position Title: Case Manager

Department: Homeless Services

Reports to: Program Director, Community Supportive Housing

Employment Status: Full-Time

FSLA Status: Exempt

Salary: $53,000 - $65,000 annual

 

The Community Supportive Housing Program provides permanent supportive housing to formerly homeless individuals and families in subsidized scattered-site apartments using the Housing First model. The program is designed to lower barriers to housing so that highly vulnerable and chronically homeless individuals and families can gain permanent housing. Supports from the Case Manager include coordination of care, life skills development, and tenancy support. Referrals to outside providers for assertive community treatment and other behavioral health services are also integrated with the support HIP provides. The Case Manager is responsible for a caseload of about 20 residents with whom they must meet at least twice per month.

 

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Works with the Program Director and multidisciplinary team in a manner that is trauma-informed to ensure residents have the individualized support they need to maintainhousing.
  • Build rapport and meet with clients in order to assess their on-going needs and to makereferrals for services that will help them maintainhousing.
  • Provideawidevarietyofclientservicessuchasreferralsandresources,lifeskillstraining,health maintenance, medication monitoring, transport to essential appointments,etc.
  • Update client charts and files regularly, documenting every client communication. This information must be entered into a Homeless Management Information System (on-lineHMIS database).
  • Meet with apartment managers and landlord to ensure stability of client in theresidential environment and negotiate new leases on an annual basis. Complete annual certification documents for theCounty.
  • Conduct home inspections minimum once permonth.
  • Maintain communication with Montgomery County, landlords, and buildingmanagement companies; and assist with required recordkeeping.
  • Attend meetings held by the County including monthly teaming meetings, and asneeded, Housing Prioritizationmeetings.
  • Attend weekly supervision and staffmeetings.
  • Participate in Interfaith Works meetings, including monthly Case Management &Vocational meetings, and otheractivities.
  • Other duties asassigned.

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)

  • Bachelor's degree in social services related field. Master's degree in Social Work, Counseling,or Psychologypreferred.
  • Two years direct service experience working with individuals who are homeless and have disabilities, which include mental illness, substance use disorders, medical conditions,and cognitiveimpairments.
  • Understanding of trauma-informed care and the Housing Firstmodel.
  • Experiencewithcrisisintervention,includingassessingforandrespondingtoindividualswith suicidal and homicidalideation.
  • Ability to manage multiple responsibilitiesefficiently.
  • Ability to work independently and with a team, collaborating with other Interfaith Works programs anddepartments.
  • Excellent written and oral communicationskills.
  • Proficiency with Microsoft Windows and Office365, in addition to Outlook, Word, Excel,and PowerPoint.
  • Clean driving record mandatory (Case Manager may have to transport clients toappointments).

Bilingual English/Spanish preferred.
