POSITION SUMMARY

We are pleased to offer the opportunity to work as a Case Manager Coordinator within the DuPage County Department of Community Services. This position acts as a lead worker to employees engaged in providing professional assistance to County residents through the coordination of services.

This position is grant funded.

RESPONSIBILITIES

  • Serves as a lead worker to employees and conducts client intake and assessment
  • Trains and guides staff on the intake and eligibility and service delivery process
  • Oversees case managers in prioritizing, decision-making, and completion of case load
  • Instructs staff on grant and documentation requirements
  • Delegates work and answers questions from staff
  • Completes an assessment of the needs of clients
  • Determines and approves eligibility and appropriateness of services, authorizes and tracks payments for programs
  • Develops a comprehensive case plan for individual clients, which includes writing appropriate goals and means in which to obtain those goals to achieve a greater self-sufficiency
  • Reviews and discusses plans and program options with clients
  • Assists clients and families in decision-making process
  • Discusses expectations with clients and families
  • Provides direct assistance to individuals with varying backgrounds and problems
  • Provides specific resource information to County residents and acts as an advocate when needed
  • Promotes awareness of services
  • Cooperates with other agencies in locating assistance for clients in areas where services are currently not provided
  • May lead or attend committee meetings within the department or partnering groups and agencies
  • Evaluates information and makes decisions within established policies and standards
  • Provides in-home assistance to promote services
  • Prepares statistical reports and other mandated paperwork

REQUIREMENTS INCLUDE

EDUCATION and/or EXPERIENCE

Completion of a Bachelor's degree in Human Services, Social Work or a related field, and three years of Social Service experience which include acting as a lead worker; or, an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as those listed above.

CERTIFICATES, LICENSES, REGISTRATIONS

Must possess and maintain in good standing a valid Illinois drivers license.
